General medical and psychosocial history

• Questions are asked about physical health, emotional health, and medication use. Patients with depression, anxiety, or other psychiatric or psychological co-morbidity will need additional assessment, support and monitoring;
• A psychosocial history is obtained that includes information regarding: living arrangements, family/social support, family obligations, work and financial status.
