Case Study 1

Jackie: 42 year old female with type 1 DM (since age 6). Jackie is presenting complaint is painful feet according to the clinic admin staff. She has been a Type I diabetic since childhood. She has been experiencing some symptoms of peripheral neuropathy but there were no visible signs of circulatory changes.

Case Study 2

Kevin is a 30 year old male who comes to the NP clinic feeling very anxious during exam week. He is in his last year of graduate course work and he is relying on high grades to keep his scholarships. He is complaining of loss of sleep, irritability, palpitations, and difficulty concentrating.

Case Study 3

Joshi Kamakani – 70 year old male with metastatic prostate cancer. Joshi is a retired engineer that the Palliative Care home care team and the NP has been looking after him at home for the last two months. Joshi was diagnosed with inoperable prostate cancer three years ago and has been treated with ablative hormone therapy.

Case Study 4

Caleb is a 42 year old male presents in a community clinic with low back pain radiating down right leg. He presents recent onset back pain with radiation to right ankle. There is no Primary Care access. He is in need of work-up and coordination of care. He was seen in the ER today. No imaging was done, and no evidence of disc prolapse was found on the exam. He was sent home with instructions to follow up with local Family Health Team.

Case Study 5

73 year old woman presents as a new patient in a primary care setting, registered to NP. She has been taking lorazepam for anxiety/poor sleep since husband’s death 10 years ago. Normally on 1mg hs, but increased poor sleep x 2-3 months, self-increased dose to 2mg. She sometimes takes during the day for anxiety with glass of scotch. Other drugs include candesartan and paroxetine (for depression).

Case Study 6

The Little family consists of Hannah-mom, Liam-dad, Rachel 17, Owen 14. They recently went on welfare due to health issues and had to move from a house to an apartment. The move required them to obtain a new PCP. They have been rostered into the NPs clients at the local community health center. During interaction Owen interrupts a few times, has poor eye contact, and looks away at his phone often.
